Introducing Transition House
A brand new model to address the mental. This new program offers families other alternatives to the crisis center and jail. The mission of Transition House is the act of the mediator with families and enable rapid assessment, advocacy and response options. Not only does Transition House offer immediate clinical outreach and support in the comfort of one's home, the individuals who are are isolated and at risk within the home are offered a variety of rehabilitation choices that avoid immobilization. Furthering the alternative, Transitions House can offer accommodations that can include daily meals, staff support, respite as needed, advocacy, education and 6-12 month clinical oversight. The focus is to help them with future housing options and mental health supportive services centered around preparing them for independent living.
