Our team works with the social worker at the hospital to create a safe, motivating life plan whether they have been hospitalized for a mental health reason or medical circumstance.
The discharge begins before they leave the hospital and our liaison works with the discharge team to access community resources, link our House Calls team to help carry out the plan and work with the family to assess the progress for refinements.
