House Calls Total Wellness Recovery Coach will work with the discharge team at the hospital to make sure upon arriving home there is support and accountability.
- the Coach will attend court dates and advocate on behalf of the client;
- write letters of recommendation;
- help prevent an admission by facilitating family meetings and explore resources such as relaxation specialists and fitness trainers;
- recreation therapists to teach healthy options.
